Begin with Section 1, where you will enter your personal and employment information. Fill in your first and last name, middle initial, address, city, state, zip code, email, and phone number.
Provide your CA ECE Workforce Registry ID number. Ensure that your registry account is updated to reflect your current employment status.
Indicate whether you work for the San Francisco Unified School District (SFUSD) by selecting 'Yes' or 'No'. Specify your current title and check the age group(s) you primarily work with.
In Section 2, have a director or site supervisor complete the employment verification section. They will confirm if you work directly with children for at least 20 hours a week and if your program receives ELS or PFA funding.
Complete the W-9 form by entering your taxpayer identification number (TIN) and signing to certify that all information provided is accurate.
