What is an input/output chart? This chart (also known as a frequency-volume chart or bladder diary) is used to assess how much fluid you drink, to measure your volume, to record how often you pass over 24 hours and to show any episodes of incontinence (leakage).
Measuring Fluid Intake. This includes all forms of fluid that the patient consumes or receives over a given period (typically measured over 24 hours). For each of these sources, measure the volume in milliliters (mL) and sum them to get the total fluid intake for the designated time period.
