  4. If you are under 18, provide the full name of your parent or legal guardian.
  5. Indicate your ethnicity in the Equal Opportunities section by ticking the relevant box that best describes you.
  6. In the Medical Conditions section, disclose any illnesses or disabilities that may require support if offered a place.
  7. Prepare your Personal Statement on one side of A4 (500-550 words) explaining why you wish to train at Fonact and attach it to your application.
  8. Specify how you plan to meet training costs and living expenses in the Fees & Living Expenses section.
  9. Indicate how you heard about Fonact.
  10. Sign where indicated as both candidate and parent/guardian if applicable. Include the date of signing.
  11. Ensure all required documents such as your personal statement and resume are included before scanning and sending to admin@fonact.com.
