Provide your mailing address, including Apt/Bldg, Street, City, State, and Zip Code. If outside the U.S., include Province/Country.
Complete the fields for the name under which postgraduate training was completed and the hospital where this training occurred.
Sign item 7 to give permission for the hospital to complete Section II and return it directly to the New York State Education Department.
Once completed, ensure that the hospital's Director of Medical Education or Department Chair fills out Section II and returns it directly to the specified address.
