Proceed to each section where you will gather information from informants. For each category (visual, auditory, etc.), ask specific questions about what stimuli the student prefers.
In each response field, input detailed answers based on the informant's feedback. Encourage them to provide examples and probe deeper into preferences.
After collecting all responses, select stimuli that can be presented or withdrawn during activities. Document these on a 3” x 5” index card as instructed.
Finally, rank order the selected stimuli based on their importance and list them in the designated area at the end of the form.
