AF Form 1786 is the Application for Appointment to the USAF Academy Under Quota Allotted to Enlisted Members of the Regular and Reserve Components of the Air Force. To compete for appointment, you must complete and submit the Pre-candidate Questionnaire and an AF Form 1786.
