Begin by filling out Section I – Student Information. Print your full name as it appears on your passport, including surname, first name, and middle name.
Select one of the certification options regarding your enrollment status. Ensure you circle either 'FIRST SEMESTER' or 'FIRST YEAR' if applicable.
Provide your program start and end dates in the designated fields. If you are a graduate, include your graduation date and attach your diploma/degree.
If you have participated in a Cultural Exchange Visitor Program before, fill in the SEVIS ID number and program details as required.
Sign and date the form at the bottom of Section I to certify that all information is accurate.
In Section II, an authorized university representative must complete their portion. They should verify your enrollment status and provide their signature on official letterhead if necessary.
