3. Fill out the 'General Customer Information' section. If you are a private customer, enter your name instead of the company name. Provide your company name, address (street, city, ZIP code), billing address, phone number, and email.
4. Indicate if you are a subsidiary or division of another company by answering the relevant question and providing the parent company's legal name and address if applicable.
5. Complete the 'Bank Reference' section with your bank's name, contact person, address, phone number, account number, routing number, IBAN, and SWIFT code.
6. Provide your European VAT registration number and/or Federal Tax ID if applicable.
7. List three trade references by providing their company names, addresses, contact persons, phone numbers, faxes, and emails.
8. Specify the requested credit amount and currency for payments (USD or EUR).
9. Review and authorize the financial release authorization section before signing and dating the form at the bottom.
