The Edinburgh Postnatal Depression Scale (EPDS) is one of the most widely used screening instruments for assessing symptoms of perinatal depression and anxiety [3, 4]. It assesses emotional experiences over the past seven days using ten Likert-scale items (See Additional file 1).
A score of 10 to 12 on the EPDS or 5 - 14 on PHQ-9 is a sign of possible depression and may not require immediate referral; use your professional judgement and provide interventions described in the next section. Any positive score on item #10 on the EPDS or #9 on the PHQ-9 requires a referral.
The EPDS focuses on psychological manifestations and excludes the three physical symptoms of major depression (fatigue, changes in weight or appetite, and psychomotor arousal/delay). The PHQ-9 not only screens for depression but also shows its severity (Kroenke et al., 2001).
The PHQ 2 and 9 should be completed by the patient, usually in the waiting room, and then scored by a staff person. Often administrative staff or medical assistants score this form and subsequently enter the score into the electronic health record.
All women should complete the EPDS at least once, preferably twice, in both the antenatal period and the postnatal period (ideally 612 weeks after the birth).
