1. Click ‘Get Form’ to open the Application Form - Initial Review in the editor.
2. Begin by filling in the MC IRB Protocol Number and Date of IRB Review at the top of the form. This information is essential for tracking your application.
3. In the PROJECT TITLE section, provide a clear and concise title for your research project. Next, fill in details about the PRINCIPAL INVESTIGATOR, including name, department, degrees, campus phone number, mailing address, and email address.
4. Complete the FACULTY SPONSOR section with similar details as above. Ensure all co-investigators are listed accurately.
5. Select the appropriate research category (Category I, II, or III) and check all relevant funding sources. If federally funded, include contact information for certification of IRB approval.
6. Provide detailed descriptions in sections regarding data collection sites and participant recruitment procedures. Attach any necessary documentation as specified.
7. Finally, review all sections for completeness before submitting your application via email to MCIRB@mercy.edu.
