1. Click ‘Get Form’ to open the Annual Leave Application Form in the editor.
2. In Section A, enter your full name, host employer, ID number, contact information, trade, and year of trade by ticking the appropriate box.
3. Move to Section B and fill in the dates for your annual leave: first day, last day, and first day back at work. Provide a reason for your leave in the designated space.
4. In Section C, read and acknowledge the employee declaration regarding insufficient leave. Sign and date this section.
5. Section D requires your host employer's authorization. Ensure they fill in their name, signature, and date.
6. Finally, Sections E is for office use only. This will be completed by administration staff after submission.
