How to use or fill out letter recommendation with our platform

- Begin by filling out the applicant's name and address at the top of the form. This identifies who the recommendation is for.
- In Section IV, list the names, phone numbers, and addresses of four individuals who will provide recommendations. Ensure you check their relationship to you as a practice management professional, past or present employer, or peer.
- Each recommender should complete their section thoroughly, including their relationship to you and how long they have known you. Encourage them to be candid and specific in their responses.
- Remind your recommenders that they must sign and date the form within 60 days of submission for it to be valid. They should then email or fax it directly to the CVPM Board.
