  5. Document any reimbursements received for destroyed property. Fill in amounts from insurance or relief agencies on Lines 2a and 2b. Check the box if you were not reimbursed by your employer or disaster relief agencies.
  6. Calculate total reimbursement on Line 2c by adding Lines 2a and 2b. Then subtract this from Line 1 to find your total unreimbursed property loss on Line 3.
  7. Enter the state sales tax rate (typically 4%) on Line 4 and calculate your total state sales tax refund due by multiplying Line 3 by Line 4 on Line 5.
