An unincorporated organization that has tax-exempt status and then incorporates must reapply for California tax-exempt status.
Mail form FTB 3500 to: EXEMPT ORGANIZATIONS UNIT MS F120, FRANCHISE TAX BOARD, PO BOX 1286, RANCHO CORDOVA, CA 95741-1286 .
Purpose. Use form FTB 3500A, to obtain California tax-exempt status, if the organization has a federal determination letter granting exemption under IRC Sections 501(c)(3), 501(c)(4), 501(c)(5), 501(c)(6), 501(c)(7), or 501(c)(19).
Form 3500A is an abbreviated two-pages, in comparison to the rather lengthy Form 3500 previously required.

Who is Required to File Form 199? All nonprofits that have received state tax-exempt status from the California Franchise Tax Board, whether incorporated in California or elsewhere, must file Form 199 each year. In addition, any nonexempt charitable trust as described in IRC Section 4947(a)(1) must file, as well.
