To obtain the form, you must log into your WES My Account. Then, you will be able to download your form from the Required Documents section of your account.
Transcripts should be postmarked from the university in their original out-going envelope (the envelope should not be written upon) and must be received within 45 days of their issue date. Transcripts should be on official school transcript paper. Photocopies are not permitted and are never considered official.
Student Records and Transcripts For student records from elementary and secondary schools (K-12), contact the appropriate school and district office. The Utah State Board of Education provides a schools directory , opens in a new tab online.
Transcript information is obtained from your high school. However, if you graduated from a high school which later merged or closed, the Management Information Services department can access the Louisiana Transcript Program to print copies.
