Service evaluation forms 2026

1. Click ‘Get Form’ to open the service evaluation form in the editor.
2. Begin by entering the student’s name and ARHS ID# in the designated fields at the top of the form.
3. Fill in the name of the agency where the service was performed, along with the start and completion dates of the service.
4. Calculate and enter the total number of hours worked between these two dates in the provided field.
5. For each performance category (Relations with others, Attitude, Dependability, Quality of Work, Attendance and Punctuality), circle the number that best describes the student's performance on a scale from 1 to 5.
6. Use the comments section to provide additional feedback. If more space is needed, feel free to use the other side of the form.
7. Finally, have the evaluator sign and date the form, and print their name and title along with their daytime phone number.
