The Form 1 is a document that every corporation in Ontario must file with the Ministry of Government and Consumer Services (MGCS) within 60 days of incorporation. It is an important document that provides the government with basic information about your corporation, such as its name, address, and directors.
At the end of the 72 hours permitted by a Form 1, the person must either be released, be admitted as a voluntary patient, or continue to be held as an involuntary patient with a certificate of involuntary admission (Form 3).
What is an Application for a Psychiatric Assessment (Form 1)? A doctor completes a Form 1 if certain criteria are met, such as the doctor having reasonable cause to believe you are suffering from a mental illness that will likely result in serious harm to yourself or others.
Once signed, the Form 1 is effective for seven days and provides authority to transfer the individual to a psychiatric facility where they may be detained, restrained, observed and examined for no more than 72 hours.
The detention may be for no more than 72 hours. In that time, a different physician must assess the condition of the patient. Any patient detained involuntarily is entitled to a hearing before the provinces Consent and Capacity Board. The patient has a right to retain counsel.

What is the difference between a Form 1 and Form 2? Unlike a Form 1, a Form 2 does not allow someone to be detained for 72 hours.
