Indicate your status by checking the appropriate box: Filer, Filing Agent, or Training Agent. If you are an individual, also check the 'Individual' box.
Proceed to Part II if applicable. Enter the Filer's Tax Number or Federal Identification Number. Remember not to enter a Social Security Number here.
In Part III, provide contact information for receiving EDGAR-related inquiries and access codes. Include a valid email address for notifications.
Complete Part IV with account information if you are a filer or filing agent. This includes identifying who should receive billing invoices.

Creating Form IDs for internal database Provide a unique identifier in the Form ID field. Select Internal Database as the Destination type. Select desired specifications in the Form Response Macro Permission field. Optional: Select the Scheduled checkbox in the Notifications section to receive periodic responses.
How do I get a form ID?
Open your web browser and navigate to the webpage where the form is located. Right-click on the desired form element to find its ID. Select Inspect or Inspect Element from the options presented. In the developer tools panel that appears, locate the highlighted code corresponding to the form element.
