Select the type of elevator by checking appropriate boxes for Passenger, Freight, or Conveyor. Mark all applicable uses such as Chair Lift or Platform.
Complete the Elevator Details section by entering information about the manufacturer, model, code cycle year, serial number, license number, capacity, number of landings, maximum passengers, and speed.
Answer compliance questions regarding CDP and CAT-5 inspection requirements. List any discrepancies if applicable.
In Parts Details, provide information about counterweight material and condition. Indicate rope condition and whether there is a machine room.
