2. Begin by entering your full name in the designated fields for Last, First, and M.I. This ensures that your application is correctly identified.
3. Fill in your Permanent Address, ID Number, Phone Number, and Email Address (using a non-TU email account) to provide necessary contact information.
4. Indicate the effective semester and year for your leave by selecting either Fall or Spring Semester and entering the corresponding year.
5. Specify your expected date of return by repeating the semester and year selection process.
6. Confirm your registration status by checking either 'am' or 'am not' currently registered for courses in the upcoming semester.
7. Provide a reason for your temporary withdrawal in the designated space to clarify your request.
8. Sign and date the form at the bottom to validate your application before submitting it to obtain necessary signatures from your academic advisor, Program Director, and Department Chair.
