2. Begin by completing one form per child. Fill in the Student Information section, including the student’s name, grade completed, date of birth, and address.
3. Provide details about any illnesses, allergies, or medical conditions that need to be noted.
4. Enter the contact information for the parent or guardian, including cell phone number and email address.
5. List an emergency contact with their name, number, and relationship to the student.
6. Select classes by filling in the class slots. Remember that tuition is $25 per class; Junior Rough Riders members receive a discount.
7. Calculate and enter the total tuition enclosed at the bottom of the form.
8. Complete the release form by signing and dating it to grant permission for emergency medical treatment.
