Oklahoma State Law allows immunization exemptions for medical, religious, or personal reasons. Parents who opt not to immunize their children will be responsible for providing an approved exemption certificate to their childs school. ALL exemptions expire at the end of the students 6th grade year.
The medical exemption requires a signature from a licensed physician in the state of Oklahoma. The religious exemption requires a signature from a parent or a religious leader. A personal belief exemption requires the signature of a parent and a brief summary of the objection.
If a Vaccination Exemption is needed: Oklahoma law allows exemptions for medical, religious or personal reasons.
Oklahoma state law requires that all new students attending Oklahoma colleges and universities for the first time provide proof of immunization for: Three shots of hepatitis B. Two shots of measles-mumps-rubella (MMR) and. Tuberculosis Testing for all incoming international students.
