The DEA Form 106 can be completed via Theft/Loss Reporting Online (TLR). In order to better track controlled substances and listed chemical products reported as lost or stolen, DEA uses of the National Drug Code (NDC) number.
Normally, under the CSA, the distribution of a schedule I or II controlled substance must be pursuant to an order form that complies with the DEA regulations. The DEA regulations provide that either a DEA Form 222 or its electronic equivalent is required for each distribution of a schedule I or II controlled substance.
IMPORTANT NOTICE: Only those persons registered with and authorized by DEA to handle controlled substances may utilize/submit this form.
The DEA-107 is for reporting any theft or loss of Listed Chemicals. Use this form if: You are reporting the theft or loss of a listed chemical.
Retail pharmacies can only request this form from the DEA once a year.

When a pharmacy is transferring Schedule 1 or 2 controlled substances to another pharmacy, the pharmacy receiving the drugs must complete DEA Form 222. When transferring any other controlled substance (Schedule 3-5), a pharmacy must document the following: Drug name, dosage, quantity, and strength. Date of transfer.
The strength only needs to be on the prescription if the medicine is available in more than one strength. The total quantity must be written in the number of dosage units, e.g. ampoules, tablets, capsules, patches, millilitres, etc (in both words and figures). It must be signed by the prescriber. It must be dated.
The purchaser fills out the form which has their name, address and DEA number. They list the drug name, strength, form and quantities desired. The name, address and DEA number of the supplier/distributor is documented.
Controlled Substance Recordkeeping. All controlled substance records are required to be retained for two years following full use, transfer, or disposal. These records include purchasing and shipping records, inventories, usage and disposal logs and disposal paperwork.
DEA Form 41 is used to request permission from the DEA to destroy controlled substances. The only time University researchers use Form 41 is after controlled substances have been lost due to accidentally spilling or breaking a container.
