4. In Part 2, select all applicable options for Direct ACH Draft/Credit of Lender Account. You can check multiple boxes as needed.
5. For Part 3, fill in the Bank Name, Mailing Address, and Account details including the ABA Number and Account Number. Ensure an authorized representative signs and dates this section.
6. If applicable, list any additional Seller/Servicer Numbers in Part 4 using the same information from Parts 2 and 3.
7. Once completed, submit your form via email or fax to Fannie Mae as instructed at the end of the document.
