Proceed to the Payment Information section. Select your category (Physician, Nurse Practitioner/Physician Assistant, Resident/Trainee) and enter the appropriate fee.
If paying by credit card, fill in the name as it appears on the card and select your card type (Discover, MasterCard, Visa).
Complete your credit card number and expiration date. Don’t forget to provide your signature at the bottom of this section.
