The Support Needs Index (SNI) is the overall intensity of support needs for the individual in all activity subscales in Section 2 of the SIS-A. What this means: The SNI is a way to compare an individuals support needs related to other individuals with intellectual and developmental disabilities.
The SIS Support Needs Index provides a composite score that reflects a persons overall intensity of support needs relative to others with developmental disabilities. The Support Needs Profile is a graphic plot that provides a visual graph or pattern of a persons support needs across all six life activity domains.
A Supports Intensity Scale (SIS) assessment determines the daily supports a person with intellectual and development disabilities needs to live as independently as possible in the community. Everyone enrolled in the Innovations Waiver will complete a SIS assessment.
What does SIS measure? The Support Needs Scale is the second section of SIS and consists of 49 life activities that are grouped into six subscales: Home Living, Community Living, Life-long Learning, Employment, Health and Safety, and Social activities.
