Mild: This level means you meet the criteria for diagnosis, but your symptoms dont go much beyond the minimum criteria. Moderate: This means your symptoms cause noticeable disruptions in your work or social interactions. Severe: This means your symptoms seriously affect your work and/or social life.
The ADHD Rating Scale-IV is completed independently by the parent and scored by a clinician. The scale consists of 2 subscales: inattention (9 items) and hyperactivity-impulsivity (9 items). If 3 or more items are skipped, the clinician should use extreme caution in interpreting the scale.
To meet criteria for ADHD there must be at least 2 items of the performance set in which the child scores a 4, or 1 item of the performance set in which the child scores a 5; ie, there must be impairment, not just symptoms, to meet diagnostic criteria. The sheet has a place to record the number of positives (4s, 5s).
A standardized measure called a T-score helps your doctor compare your results. When your T-score is less than 60, it usually means you dont have ADHD. A score higher than 60 may indicate ADHD. And a T-score higher than 70 means your ADHD symptoms are more serious.
Scores of 2 or 3 on a single symptom question reflect often-occurring behaviors. Scores of 4 or 5 on performance questions reflect problems in performance. The initial assessment scales, parent and teacher, have 2 components: symptom assessment and impairment in performance.

In comparing diagnostic performance of ADHD scales, investigators, led by Ling-Yin Chang, PhD, Taipei Medical University, in Taiwan, found that the Child Behavior ChecklistAttention Problem (CBCL-AP) scale, the Conners Rating ScaleRevised (CRS-R), and the Conners Abbreviated Symptom Questionnaire (ASQ) came out on
Low: 9 or less. Mild to Moderate: 10-13. High: 14-17. Very High: 18 or more.
The conners rating consists of a number of multiple-choice questions (59 questions for the Teacher rated conners, 80 the Parent rated conners), these are known as items. For each item there are four multiple choice answers scored 0 to 3. So each item can have a score of 0 3.
