Begin by entering the insured’s claim ID and name, along with the informal caregiver’s name at the top of the form.
For each date of service, enter one date per line. Fill in the time in and time out, ensuring to specify a.m. or p.m., rounding to the nearest quarter hour.
Complete the total hours worked, approved hourly charge, and daily total for each service date.
Input the total reimbursement amount requested at the bottom of this section.
Mark an ‘X’ next to each activity of daily living service provided during that time period.
Enter check or transaction numbers corresponding to each date of service and attach appropriate proof of payment as outlined in the instructions.
Ensure both the informal caregiver and insured (or their legal representative) sign and date the invoice after services are rendered.
