6. In section 5, note any allegations you agree with from the received documents.
7. In section 6, specify any allegations you disagree with and provide reasons.
8. State any requests you have for the Court in section 7.
9. Sign and date the form in front of a notary or Clerk of Court.
10. File the original with the Clerk of Court and retain a copy for yourself. Mail a copy to the opposing party and complete an Affidavit of Service by Mail (UJS-338) to file with the Clerk of Court.
