The purpose of Form 5498-SA is to report how much money is contributed to an HSA, Archer MSA or MA MSA over the course of a tax year. In the case of HSAs and Archer MSAs, the tax year extends to April 15 of the following year.
Your IRA provider will provide you with your Form 5498 if you are eligible to receive one. They may send it to you via mail or digitally. You may also be able to find it on their website.
The IRS Form 1099-SA is used for reporting HSA distributions; the IRS Form 5498-SA is used for reporting contributions. If you did not have contributions during the year, then you will not see the IRS Form 5498-SA. Although HSAs have triple-tax advantages, you are required to report distributions to the IRS.
