The USPS Form 1583, or Application for Delivery of Mail Through Agent form, is a document that authorizes a Commercial Mail Receiving Agency (CMRA) to handle and receive mail on a customers behalf.
What Happens If I Dont Submit USPS Form 1583? If you fail to submit this form, your CMRA or virtual mailbox provider wont be able to receive mail on your behalf. This could lead to returned or undelivered mail, causing disruptions to your personal or business correspondence.
This form will be valid as long as you are using the same address and recipients. If the address or recipients change, then you will need to fill out a new one for the new address or new recipient.
You must submit two (2) forms of photo identification with each completed Form 1583. To upload Form 1583 and supporting documents, select the form or document on your computer and click Upload Document. Please note: you must also upload images of your photo identification. Replace (####) with your suite number.
