1. Click ‘Get Form’ to open the subscriber information form in the editor.
2. Begin by entering your Subscriber Name. You can optionally include your title (Mr., Mrs., Miss, Ms.) for personalization.
3. Fill in the License and Association Information section. Select your License Type from the options provided: Broker of Record, Salesperson, Associate Broker, or Not Licensed. Enter your License Number and License Expiration Date.
4. Provide your Home Address details including Home City, State, and Zip + 4. This ensures accurate communication.
5. In the Office Information section, enter your Office Name and Office City/State. Include the Office Broker/Participant's name as required.
6. Input your Phone Numbers by selecting the type (Home, Mobile, Office) and entering the corresponding numbers.
7. Lastly, provide your Email Address and any Other MLS Memberships if applicable. Review all entries for accuracy before submission.
