2. Begin by entering the Analyzer Serial #, Kit Lot, Expiration Date, and Date Opened in the designated fields at the top of the form.
3. Fill in the QC Lot # and QC Expiration Date to ensure accurate tracking of your quality control samples.
4. In the Controls section, input the Target (µg/dL) and Tolerance (±) for both Level 1 and Level 2 controls.
5. Record the Date, QC Level, and Result (µg/dL) for each control level. Ensure that you check whether each control is within range by selecting 'Yes' or 'No' as appropriate.
6. Use the Comments / Corrective Actions section to note any observations or necessary actions based on your results.
7. Finally, add your initials at the bottom of the form to validate your entries before saving or sharing.
