Student Reassignment Request Form - Updated February 2020-2025

1. Click ‘Get Form’ to open it in the editor.
2. Begin by entering the date received by the Student Reassignment Office at the top of the form.
3. Fill in your Student ID Number, followed by the student's name, grade, designated school, current school, and requested school for the upcoming school year.
4. Provide parent/guardian information including name, daytime phone number, email address, and current address.
5. Select a reason for reassignment from the options provided: Relocation, Buy/Build/Lease, or Other. Follow the corresponding instructions for each option.
6. Complete any additional sections required based on your selected reason for reassignment. Ensure all necessary signatures are obtained where indicated.
7. Review all entered information for accuracy before submitting your completed form to student_reassignment@hcpss.org.
