A Prospective Payment System (PPS) is a method of reimbursement in which Medicare payment is made based on a predetermined, fixed amount. The payment amount for a particular service is derived based on the classification system of that service (for example, diagnosis-related groups for inpatient hospital services).
To figure out how much money your hospital got paid for your hospitalization, multiply your DRGs relative weight by your hospitals base payment rate. Heres an example with a hospital that has a base payment rate of $6,000 when your DRGs relative weight is 1.3: $6,000 X 1.3 = $7,800.
A prospective payment system creates an incentive structure that rewards quality care since providers receive a set amount regardless of how much or how little it costs them to provide the service. This helps drive efficiency instead of incentivizing quantity over quality.
Under the IPPS, Medicare payment for hospital inpatient operating and capital-related costs is made at predetermined, specific rates for each hospital discharge. Discharges are classified ing to a list of Medicare Severity Diagnosis Related Groups (MS-DRGs).
NOTES: PPS is prospective payment system. The abbreviation PPS followed by a number indicates a particular year under the system, e.g., PPS 1 is the first year of PPS.

CMS issued the CY 2024 Hospital Outpatient Prospective Payment System (OPPS) and Ambulatory Surgical Center (ASC) Payment System final rule that announces Medicare payment rates for hospital outpatient and ASC services. See a summary of key provisions effective January 1, 2024.
The IPPS pays a flat rate based on the average charges across all hospitals for a specific diagnosis, regardless of whether that particular patient costs more or less. Everything from an aspirin to an artificial hip is included in the package price to the hospital.
The IRF PPS will utilize information from a patient assessment instrument (IRF PAI) to classify patients into distinct groups based on clinical characteristics and expected resource needs. Separate payments are calculated for each group, including the application of case and facility level adjustments.
