Form W-7 asks for the following information: the reason the applicant needs an ITIN. the applicants name, mailing address and, if different, foreign address. the applicants birth date and location of birth. the country where the applicant is a citizen. the applicants foreign tax ID number, if they have one.
Mistyped name: Double-check your name for accuracy, including any middle name or suffixes. Inaccurate date of birth: Ensure the date of birth matches your passport or other identification documents.
