2. Begin by filling out the Payroll Manager section at the top. Enter your Company Code, Company Name, Employee File Number, Payroll Manager's Name, and Signature.
3. Attach a voided check for each checking account you wish to enroll. If using a savings account, obtain the Routing/Transit Number from your bank.
4. In the Employee Information section, provide your name and Social Security Number. Ensure you sign and date this section.
5. For Account Information, specify the Bank Name, Routing Transit Number, and Account Number for each account where you want deposits made. Indicate whether it’s Checking or Savings and how much to deposit.
6. Review all entries for accuracy before submitting the form to your Payroll Manager.
