Once you have notified your employer, they will give you the retirement application form AW8. You must complete parts 6 to 14 of this form. You must then return the form to your employer for them to complete parts 1 to 5. Your employer will then submit your application to NHS Pensions on your behalf.
Please return this form together with the certificates required to: NHS Pensions, PO Box 2269, Bolton, BL6 9JS. Please enter your Scheme membership number. It is an eight digit number.
The member should complete Parts 7 to 15 of the form with their personal details and return the form to their employer together with any relevant certificates. Only original or certified copies of original documents are acceptable.
