Analytics request form template 2026

1. Click ‘Get Form’ to open the analytics request form template in the editor.
2. Begin by filling out the 'Principal Researcher' section. Select your title (Dr., Mr., Mrs., Ms.) and enter your organization, first name, last name, department, job title, mailing address, city/town, province, postal code, phone number, extension, and email.
3. In the 'Research Information' section, provide the research title and research question(s). Include a brief description of your research and select the type of service requested: either access to data or analytics & reporting.
4. If applicable, indicate whether you will import externally collected data by selecting 'Yes' or 'No', and provide details if you selected 'Yes'.
5. Fill in the estimated start and end dates for your research. Check all applicable purposes for the research output.
6. In the 'Funding' section, specify your financial resources for research. Indicate if funding is pending or has been obtained.
7. Review all entered information for accuracy before submitting. Once complete, submit the form via email to das@ices.on.ca.
