- Enter the 'Date of Purchase', 'Invoice No.', and 'Gross Amount of Sale Excluding Tax'.
- Indicate whether you requested a sales tax refund from the dealer by selecting 'Yes' or 'No'. If yes, provide the date of your request.
- Complete sections regarding any tax paid to the dealer and details about items purchased. Ensure to attach copies of your request and proof of mailing or delivery.
- Answer questions about whether the dealer refunded any sales tax and what actions they took regarding your request.
- Finally, sign and date the affidavit at the bottom. Ensure that it is notarized as required.
