- Enter the Cost/Estimated Value of the item and specify its Room/Bldg location.
- Ensure that the Loss Acknowledged By section is signed by a Principal, Department Head, or Designee.
- If applicable, provide details for Name of Donor and Donor Address for donations received.
- Finally, ensure that Acquisition Approved By is signed by an authorized individual before submission.
