4. Enter details for the Proposed Position including Grade/Step and FTE (Full-Time Equivalent) to clarify your staffing needs.
5. Provide information about the person currently filling this position or who is identified to fill it, along with the Official Start and End Dates of the position.
6. Complete the Annual Salary field and indicate if it’s a Soft Money Position. Don’t forget to include the Account Number and Position Control Number.
7. Specify the Vacancy Date and name of the Previous Employee in Vacant Position. Select Yes or No as applicable.
8. In the Justification For Request section, provide a brief explanation supporting your personnel action request.
9. Ensure all necessary approvals are obtained from Hiring Supervisor, Dean or Director, Vice President, Budget Office, and Human Resources by checking Approve or Disapprove as needed.
10. Once all signatures are in place, send your completed form to Human Resources at W40E for processing.
