Grade waiver 2026

1. Click ‘Get Form’ to open the grade waiver in the editor.
2. Begin by entering your name, ID number, and graduation year in the designated fields at the top of the form.
3. Fill in the course name and course number for which you are requesting a waiver.
4. Indicate your teacher's name and select the appropriate period for the course.
5. Check the boxes for Sem 1, Sem 2, or Full year to specify when you are requesting the waiver.
6. Review the eligibility criteria and ensure you meet them before submitting. Remember, you must be enrolled in at least four courses that count towards your GPA.
7. Sign and date the form at the bottom. Ensure your parent also signs it before submission.
8. Submit the completed form to your counselor within eight school days after the first grading period.
