2. Begin by entering your Palomar Student ID Number and your current name as it appears on file. This ensures that your records are accurately updated.
3. If you are requesting a name change, fill in the new last name, first name, and middle initial. Remember to attach a copy of a government-issued ID along with a court order, marriage certificate, or divorce decree.
4. For social security number corrections or updates, enter your new social security number and attach a signed copy of your social security card.
5. Update your contact information by filling in the new address details including street number, apartment number (if applicable), city, state, zip code, and phone number.
6. Indicate any consent cancellations by checking the appropriate boxes for information you do not wish to be released without written consent.
