1. Click ‘Get Form’ to open the employee fitness form in the editor.
2. Begin by entering your first and last name in the designated fields. If applicable, include your spouse’s name as well.
3. Indicate your classification as a PNW benefits-eligible employee or spouse by selecting the appropriate option.
4. Fill in your mailing address, phone number, and email address clearly to ensure accurate communication.
5. Provide the date of your fitness center membership purchase and select which wellness participation you engaged in: either a Wellness Panel or an Annual Wellness/Physical Exam.
6. If you participated in an Annual Wellness/Physical Visit, enter the date of that visit.
7. Attach a copy of your PNW Fitness Center membership receipt before submitting the form via email or campus mail as indicated.
