Residency affidavit Texas requirements: 2022-2023 Provide the basic data about the person who wants to execute this document. Your name, surname, and, of course, place of residence are entered. The residency affidavit (pdf) (formerly known as the HB1403 affidavit) must be filled out by applicants and current students who are not U.S. citizens or permanent residents and wish to be classified as a resident based on graduation from a Texas high school and residence in the state (part a, above).
A. Physical Residency: Submit one document from the list below to support physical presence for 12 consecutive months in Texas. Utility bills in name of the person. Texas high school transcript. Transcript from a Texas institution. Texas drivers license or Texas ID card showing origination date.
Core Residency Questionnaire Texas higher Education Coordinating Board rule 21.731 requires each student applying to enroll at an institution to respond to a set of core residency questions for the purpose of determining the students eligibility for classification as a resident.
