1. Click ‘Get Form’ to open the English Translation Verification Form in the editor.
2. In Section 1, enter your name and institution name. Check the boxes for the verified documents you are submitting, such as your Bachelor’s or Master’s degree transcripts.
3. Follow the instructions provided to ensure that your translations are accurate and complete. If needed, consult with your department for assistance in finding a verifier.
4. Once you have completed Section 1, present both the original documents and their translations to a UIUC faculty or staff member for verification.
5. The faculty or staff member will complete Section 2 by providing their name, title, department, and confirming their competency in the native language. They must also sign and date the form.
6. After verification, submit all required documents to the Graduate College as instructed.
