3. For Lines 1 through 4, input the total Oregon tax reported for each quarter using amounts from your 2014 OQ forms.
4. Calculate the total amount from all quarters and enter it in Line 5.
5. In Line 6, enter the total Oregon tax withheld as shown on your employees' W-2s or 1099s.
6. On Line 7, calculate the difference between Line 5 and Line 6. If Line 6 is larger, you owe tax; if smaller, you may have a credit.
7. Provide an explanation for any differences in the designated area on the form.
8. Sign and date your completed Form WR. Print your name and telephone number before submitting.
