5. In Part A (EPF), nominate individuals to receive your Provident Fund amount by entering their names, addresses, relationships with you, and their dates of birth.
6. If any nominee is a minor, include the guardian's details who will manage the funds until they reach adulthood.
7. Complete Part B (EPS) by listing family members eligible for widow/children pension and providing their details similarly.
8. Finally, sign or provide a thumb impression at the end of the form to validate your submission.
