How to use or fill out option designation with our platform

- For Option 2, provide the name of your Survivor Annuitant and ensure to attach a copy of their birth certificate.
- If selecting Option 3, similarly designate your Survivor Annuitant and include their details along with a birth certificate copy.
- For any Special Options, specify your payment plan details in the attached letter and ensure all information is complete before signing.
